It was one of four texts we were working with, so it wasn´t from overuse, and there were eight to ten other people in the class experiencing the same thing. The publisher ought to be ashamed of themselves. A book like this is meant to be used often as a reference for a classroom teacher. Instead it´s useless because it´s falling apart.
